Our Restorative Home Care service is different from traditional home care. So, when you need skilled help at home, an experienced Registered Nurse arrives ready to enhance your strength and independence at home with confidence.
A Consortia Care nurse cuts through the fog and uncertainty of functional decline, giving you and your family hope to manage your health at home. We truly want to help restore you, which includes connecting you to the right set of health care providers at the right time. When you call, an on-call Registered Nurse is your first point of contact. A Consortia Care nurse stands ready to answer any questions you might have about a range of community care options – both public and private.
How is a Consortia Care nurse different?
Most home care companies provide companionship and basic nursing care. A Consortia Care nurse offers the “next level” of home care support when you need it. Registered Nurses at Consortia Care are highly-trained professionals with a passion for rehabilitative nursing care. All Consortia Care nurses are also trained in key clinical practice guidelines, including those published by Diabetes Canada, CHEP, the PATH clinic and the Canadian Cardiovascular Society. Finally, our nurses are integrated with key community partners to ensure you find the care you need.
Consortia Care nurses care deeply about your needs. They are accountable to you and continually advocate for the best possible care from all home care providers.
Who will benefit from this service?
The Restorative Home Care service steps-into your care when you need extra help at home. Because people (and their families) sometimes struggle at home, we typically start by addressing key functional deficits affecting your day-to-day life.
However, our home care services do not replace emergency care or even your family doctor. If you need non-urgent medical care and cannot leave the house, your nurse can facilitate a tele-medicine consultation or case conference with your family doctor or an alternate prescriber. Your nurse, however, does not intend to replace any existing member of your team.
If you are not already attached to a family doctor or nurse practitioner, the nurse will connect you with one before your in-home consultation.
What if I am not a fit for this service?
That’s o.k. If interested and available, we encourage everyone to try to access publicly funded home care services first. Sometimes we even wait until other providers have finished their course of treatment. Everyone receives a free Meet & Greet with an RN (30 min visit, in clinic or at home) to discuss different community care options before we formally step-into your care.

Introduce your main health concerns, and develop an initial plan for further assessment and/or referral to other services or professionals. Available evenings and weekends; in clinic or at home.
RN Consult – Initial | 2 hour consult + follow-up
