Patients can self-refer to this program; however, a physician referral is required to access provincial funding for care.

Our Senior’s Mental Health program supports successful adaptation to grief, loss, and neurocognitive disorders.

With a physician referral, Edmonton seniors (age 65+) may qualify for 3 hours of funded home care with Adam, our Clinical Nurse Therapist.

This program offers:
  • Home counselling and on-call supports
  • House calls for urgent needs
  • Neuropsychiatric assessments & care
  • Nutrition support, coaching & referrals
  • Medication management
  • Supervised home exercise

Home counselling, nursing care & respite care to help seniors address social isolation, anxiety, memory loss, and depression at home.
