Rapid assessment and care for memory loss, anxiety, and depression concerns.

With a physician referral, seniors (age 65+) experiencing mental health distress may have coverage for a 2 to 3 hour home visit with our Registered Nurse Therapist.

  • Nutrition support and home exercise
  • Medication management
  • Multidisciplinary case conferencing
