A physician referral is required. Patients and family caregivers living in Red Deer, AB are welcome to call us for help facilitating referral to this program.
Rapid assessment and support for memory loss, grief, anxiety, and depression among seniors.
With a physician referral, seniors (age 65+) may have coverage for a comprehensive home visit with our Registered Nurse Therapist.
This program enhances access to:
- Comprehensive biopsychosocial care
- Urgent mental health supports
- Behaviour support planning
- Caregiver coaching and respite care
- Nutrition support and home exercise
- Medication management
- Multidisciplinary case conferencing
- Care coordination and referrals
