Rapid assessment and support for memory loss, grief, anxiety, and depression among seniors.

With a physician referral, seniors (age 65+) may have coverage for a comprehensive home visit with our Registered Nurse Therapist.

This program enhances access to:

  • Urgent mental health supports
  • Behaviour support planning
  • Caregiver coaching and respite care
  • Nutrition support and home exercise
  • Medication management
  • Multidisciplinary case conferencing
  • Care coordination and referrals
